Heads Up Physiotherapy Privacy Policy

Your privacy is important to Heads Up Physiotherapy.  This statement provides information about how we store and use your personal information.

We may ask you to provide personal details so we can deal with your query, allocate appropriate appointments and communicate effectively with you regarding your treatment.  In addition, we may provide newsletters, updates or invitations to provide feedback about our services, participate in surveys, attend events or enjoy special offers.  We may communicate with you via email, telephone, SMS or mail, according to your preference or availability.  Please indicate if you do not wish Heads Up Physiotherapy to use a particular method, or if you wish to opt out of communications not directly related to your treatment.

It is usual for us to communicate with other health care providers, regarding your care.  These are only accredited professionals or your case manager. Information to a third party, such as an insurer or lawyer is only provided on receipt of a Release of Information Document, signed by you.

We may request permission to take photos of you.  These are to assist in assessment, treatment planning and to record progress.  They are stored with your records in a secure place.  We may ask your permission to share these images with other health professionals providing your care or to use for teaching purposes.  Your help in this area is appreciated but we understand if you do not wish the images to be used for teaching.

All members of our staff are bound by a strict confidentiality agreement which extends within and beyond their time at Heads Up Physiotherapy.
